En funci\u00f3 de les caracter\u00edstiques moleculars de cada tumor \u2014mutacions en l\u2019ADN i d\u2019altres\u2014, la medicina de precisi\u00f3 vol facilitar que els pacients oncol\u00f2gics, tant adults com pedi\u00e0trics, puguin rebre un tractament personalitzat i adequat a la seva patologia. Per\u00f2, \u00e9s possible saber si un pacient pot beneficiar-se o no d\u2019un tractament, abans d\u2019iniciar-lo?<\/strong><\/p>\n\n\n\n<figure class=\"wp-block-image size-full is-resized\"><img decoding=\"async\" data-src=\"https:\/\/ibecbarcelona.eu\/wp-content\/uploads\/2023\/03\/BH3-1.jpg\" alt=\"\" class=\"wp-image-106994 lazyload\" width=\"1000\" data-srcset=\"https:\/\/ibecbarcelona.eu\/wp-content\/uploads\/2023\/03\/BH3-1.jpg 720w, https:\/\/ibecbarcelona.eu\/wp-content\/uploads\/2023\/03\/BH3-1-300x169.jpg 300w\" data-sizes=\"(max-width: 720px) 100vw, 720px\" src=\"data:image\/svg+xml;base64,PHN2ZyB3aWR0aD0iMSIgaGVpZ2h0PSIxIiB4bWxucz0iaHR0cDovL3d3dy53My5vcmcvMjAwMC9zdmciPjwvc3ZnPg==\" style=\"--smush-placeholder-width: 720px; --smush-placeholder-aspect-ratio: 720\/405;\" \/><figcaption class=\"wp-element-caption\">Els experts Albert Manzano i Joan Montero a la Facultat de Medicina i Ci\u00e8ncies de la Salut de la UB. \/Cr\u00e8dit: Universitat de Barcelona (UB).<\/figcaption><\/figure>\n\n\n\n<p>Un equip d\u2019experts de l\u2019Institut de Bioenginyeria de Catalunya (IBEC) i de la Facultat de Medicina i Ci\u00e8ncies de la Salut de la Universitat de Barcelona  ha dissenyat un dispositiu microflu\u00eddic anomenat&nbsp;<em>microfluidic dynamic BH3 profiling<\/em>&nbsp;(<strong>\u03bcDBP<\/strong>) que prediu l\u2019efic\u00e0cia del tractament del c\u00e0ncer de manera r\u00e0pida i automatitzada, utilitzant un nombre redu\u00eft de c\u00e8l\u00b7lules procedents de bi\u00f2psies i sense requerir personal t\u00e8cnic especialitzat.<\/p>\n\n\n\n<p>El treball, publicat a la revista&nbsp;<a href=\"https:\/\/www.nature.com\/articles\/s41698-022-00333-0\"><em>npj Precision Oncology<\/em><\/a>, l\u2019han dirigit Joan Montero, professor del Departament de Biomedicina de la UB i l\u2019IBEC, i Javier Ram\u00f3n Azc\u00f3n, professor de recerca ICREA a l\u2019IBEC. En l\u2019estudi, que t\u00e9 com a primer autor Albert Manzano (UB-IBEC), que es va doctorar a la UB el 2022 amb una tesi sobre medicina de precisi\u00f3 en la lluita contra el c\u00e0ncer, tamb\u00e9 hi han participat experts de la Facultat de F\u00edsica de la UB, l\u2019Institut d\u2019Oncologia Vall d\u2019Hebron (VHIO) i el Centre de Recerca Biom\u00e8dica en Xarxa de Bioenginyeria, Biomaterials i Nanomedicina (CIBER-BBN). &nbsp;&nbsp;<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>Medicina de precisi\u00f3 en la lluita contra el c\u00e0ncer<\/strong><\/h4>\n\n\n\n<p>La medicina personalitzada ha revolucionat la manera de dissenyar tractaments contra el c\u00e0ncer, que cada vegada s\u00f3n m\u00e9s efica\u00e7os. Si considerem que cada tumor \u00e9s \u00fanic i t\u00e9 caracter\u00edstiques pr\u00f2pies, poder disposar d\u2019indicadors predictius de la resposta de cada pacient al tractament \u00e9s un gran pas endavant en oncologia. El&nbsp;<em>dynamic BH3 profiling<\/em>&nbsp;(DBP) es va desenvolupar inicialment al laboratori del professor Anthony Letai \u2014el professor Montero en va ser coinventor\u2014 i el va patentar l\u2019Institut Oncol\u00f2gic Dana-Farber (Estats Units) l\u2019any 2015. Va ser un dels primers assajos funcionals provat amb \u00e8xit per predir el tractament en diversos tipus de c\u00e0ncer.<\/p>\n\n\n\n<p>Aquest sistema posa en contacte les c\u00e8l\u00b7lules canceroses amb diferents opcions terap\u00e8utiques per identificar&nbsp;<em>ex vivo<\/em>&nbsp;i de manera r\u00e0pida les que podrien ser m\u00e9s efectives per eliminar el tumor. Conceptualment, \u00e9s molt similar als antibiogrames que s\u2019utilitzen per identificar antibi\u00f2tics per tractar infeccions bacterianes.<\/p>\n\n\n\n<p>\u00abEl DBP s\u2019ha utilitzat per identificar l\u2019efic\u00e0cia de tractaments a escala precl\u00ednica i cl\u00ednica en molts tipus de c\u00e0ncer diferents, tant s\u00f2lids com l\u00edquids. Aquests estudis han fet servir l\u00ednies cel\u00b7lulars, models animals i mostres prim\u00e0ries amb una gran capacitat predictiva en tots els casos. Tot i aix\u00ed, aquest assaig encara no s\u2019ha aplicat de manera generalitzada als hospitals\u00bb, detalla el professor Montero.<\/p>\n\n\n\n<p>\u00abDe moment \u2014afegeix\u2014 diversos estudis han trobat una bona correlaci\u00f3 entre els resultats del DBP i la resposta cl\u00ednica en mostres prim\u00e0ries de leuc\u00e8mia. Quan es fa una bi\u00f2psia a un pacient, el nombre de c\u00e8l\u00b7lules tumorals obtingudes \u00e9s molt limitat, cosa que no permet fer un estudi amb gaire tractaments diferents i limita la capacitat d\u2019identificar-ne un d\u2019efica\u00e7\u00bb, explica l\u2019expert Albert Manzano.<\/p>\n\n\n\n<p>Quan es rep la mostra d\u2019una bi\u00f2psia, es dissocia per poder obtenir-ne c\u00e8l\u00b7lules individuals fent servir un tractament mec\u00e0nic i enzim\u00e0tic. Un cop processada, la mostra es filtra per obtenir-ne c\u00e8l\u00b7lules individuals que, despr\u00e9s, se sotmeten als tractaments pertinents i se sembren en el dispositiu microflu\u00eddic.<\/p>\n\n\n\n<p>\u00abGr\u00e0cies a la nostra plataforma microflu\u00eddica&nbsp;<strong>\u03bcDBP<\/strong>, que est\u00e0 dotada de petits pous per sembrar les c\u00e8l\u00b7lules, podem reduir el nombre c\u00e8l\u00b7lules requerides per provar un tractament. Es tracta d\u2019una innovaci\u00f3 decisiva per augmentar el nombre de f\u00e0rmacs que es poden avaluar\u00bb, afegeix Manzano.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong><strong>Un sistema r\u00e0pid i completament automatitzat<\/strong><\/strong><\/h4>\n\n\n\n<p>Aquest \u00e9s el primer treball en qu\u00e8 s\u2019aplica la microflu\u00eddica per fer l\u2019assaig funcional del DBP. A difer\u00e8ncia d\u2019altres versions desenvolupades fins ara, com el&nbsp;<em>high-throughput<\/em>&nbsp;DBP (Bhola et al.,&nbsp;<em>Science Signaling<\/em>, 2020), amb plaques i dispensadors autom\u00e0tics per provar centenars de tractaments, el nou dispositiu&nbsp;<strong>\u03bcDBP<\/strong>&nbsp;est\u00e0 orientat a provar tractaments&nbsp;<em>in situ<\/em>&nbsp;de manera molt r\u00e0pida \u2014cosa que evita el deteriorament de les mostres\u2014, senzilla i automatitzada, sense la necessitat de maquin\u00e0ria costosa o personal especialitzat.<\/p>\n\n\n\n<p>\u00abEl m\u00e0xim avantatge del dispositiu&nbsp;<strong>\u03bcDBP<\/strong>&nbsp;\u00e9s tamb\u00e9 l\u2019automatitzaci\u00f3 de tot el proc\u00e9s, que ajudaria a implementar aquesta metodologia funcional a escala cl\u00ednica. En conjunt, tots aquests avantatges facilitarien l\u2019adopci\u00f3 del DBP als hospitals com a assaig rutinari\u00bb, detallen els experts.<\/p>\n\n\n\n<p>\u00abHem desenvolupat aquesta nova eina amb la idea de posar-la a l\u2019abast dels onc\u00f2legs.
