A difer\u00e8ncia de la nanomedicina tradicional, que es basa en nanopart\u00edcules com a portadores de mol\u00e8cules terap\u00e8utiques, aquest enfocament utilitza nanopart\u00edcules que s\u00f3n bioactives per si soles: &#8220;f\u00e0rmacs supramoleculars&#8221;. En lloc de dirigir-se directament a les neurones, <strong>la ter\u00e0pia restaura la funci\u00f3 adequada de la barrera hematoencef\u00e0lica (BBB per les seves sigles en angl\u00e8s)<\/strong>, el &#8220;guardi\u00e0 vascular&#8221; que regula l&#8217;entorn del cervell. En reparar aquesta interf\u00edcie cr\u00edtica, els investigadors van aconseguir revertir la patologia de l&#8217;Alzheimer en models animals.<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>Nom\u00e9s 1 hora despr\u00e9s de la injecci\u00f3 observem una reducci\u00f3 del 50-60% en la quantitat d&#8217;A\u03b2 dins del cervell.<\/p>\n\n\n\n<p>Junyang Chen<\/p>\n<\/blockquote>\n\n\n\n<p>El cervell \u00e9s l&#8217;\u00f2rgan m\u00e9s car del cos, consumint el 20% de l&#8217;energia en adults i fins al 60% en nens. Aquesta energia arriba a trav\u00e9s d&#8217;un vast subministrament de sang, assegurat per un sistema vascular \u00fanic i dens on cada neurona es nodreix d&#8217;un capil\u00b7lar. El nostre cervell cont\u00e9 aproximadament mil milions de capil\u00b7lars, cosa que destaca el paper vital del sistema vascular cerebral en el manteniment de la salut i la lluita contra les malalties. Aquestes troballes ressalten el paper crucial de la salut vascular, especialment en malalties com la dem\u00e8ncia i l&#8217;Alzheimer, que es relacionen amb un sistema vascular comprom\u00e8s.<\/p>\n\n\n\n<p>La BBB \u00e9s una barrera cel\u00b7lular i fisiol\u00f2gica que separa el cervell del flux sanguini per protegir-lo de perills externs com pat\u00f2gens o toxines. L&#8217;equip d&#8217;investigadors va demostrar que enfocar a un mecanisme espec\u00edfic permet que les &#8220;prote\u00efnes de rebuig&#8221; indesitjades produ\u00efdes al cervell travessin aquesta barrera i s&#8217;eliminin en el flux sanguini correctament. En la malaltia d&#8217;Alzheimer, la principal prote\u00efna &#8220;de rebuig&#8221; \u00e9s l&#8217;amiloide-\u03b2 (A\u03b2), l&#8217;acumulaci\u00f3 de la qual perjudica el funcionament normal de les neurones.<\/p>\n\n\n\n<p>Els investigadors van utilitzar models de ratol\u00ed que estan programats gen\u00e8ticament per produir majors quantitats de prote\u00efna A\u03b2 i desenvolupar un deteriorament cognitiu significatiu que imita la patologia de l&#8217;Alzheimer. Van administrar nom\u00e9s 3 dosis dels f\u00e0rmacs supramoleculars i posteriorment van monitorar regularment l&#8217;evoluci\u00f3 de la malaltia. &#8220;<strong>Nom\u00e9s 1 hora despr\u00e9s de la injecci\u00f3 observem una reducci\u00f3 del 50-60% en la quantitat d&#8217;A\u03b2 dins del cervell<\/strong>&#8220;, explica Junyang Chen, primer coautor de l&#8217;estudi, investigador de l&#8217;Hospital West China de la Universitat de Sichuan i estudiant de doctorat del University College London (UCL).<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p><em><em>L&#8217;efecte a llarg termini prov\u00e9 de la restauraci\u00f3 del sistema vascular del cervell. El m\u00e9s rellevant \u00e9s que les nostres nanopart\u00edcules actuen com un f\u00e0rmac i semblen activar un mecanisme de retroalimentaci\u00f3 que retorna aquesta via d&#8217;eliminaci\u00f3 a nivells normals.<\/em><\/em><\/p>\n\n\n\n<p>Giuseppe Battaglia<\/p>\n<\/blockquote>\n\n\n\n<p>Les dades m\u00e9s destacades van ser els efectes terap\u00e8utics. Els investigadors van realitzar diversos experiments per analitzar el comportament dels animals i mesurar el deteriorament de la mem\u00f2ria durant diversos mesos, cobrint totes les etapes de la malaltia. En un dels experiments, van tractar un ratol\u00ed de 12 mesos (equivalent a un hum\u00e0 de 60 anys) amb les nanopart\u00edcules i van analitzar el seu comportament despr\u00e9s de 6 mesos. <strong>El resultat va ser impressionant: l&#8217;animal, de 18 mesos (comparable a un hum\u00e0 de 90 anys), havia recuperat el comportament d&#8217;un ratol\u00ed sa.<\/strong><\/p>\n\n\n\n<p><em>&#8220;L&#8217;efecte a llarg termini prov\u00e9 de la restauraci\u00f3 del sistema vascular del cervell. Creiem que funciona com una cascada: quan s&#8217;acumulen esp\u00e8cies t\u00f2xiques com la beta amiloide (A\u03b2), la malaltia progressa. Per\u00f2 una vegada que la vascularitzaci\u00f3 pot funcionar novament, comen\u00e7a a eliminar A\u03b2 i altres mol\u00e8cules perjudicioses, cosa que permet que tot el sistema recuperi el seu equilibri. Verd: vasos sanguinis de la barrera hematoencef\u00e0lica.<\/figcaption><\/figure>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>Eliminaci\u00f3 d&#8217;amiloide-\u03b2 del cervell<\/strong><\/h4>\n\n\n\n<p>En la malaltia d&#8217;Alzheimer, un dels problemes clau \u00e9s que el sistema natural d&#8217;eliminaci\u00f3 del cervell per a toxines com la prote\u00efna amiloide-\u03b2 deixa de funcionar correctament. Normalment, la prote\u00efna LRP1 actua com un guardi\u00e0 molecular: reconeix A\u03b2, s&#8217;hi uneix per lligands i la transporta a trav\u00e9s de la barrera hematoencef\u00e0lica cap al torrent sanguini, on es pot eliminar. Per\u00f2 aquest sistema \u00e9s fr\u00e0gil. Si LRP1 s&#8217;uneix a massa A\u03b2 i amb massa for\u00e7a, el transport s&#8217;obstrueix i la prote\u00efna mateixa es degrada dins de les c\u00e8l\u00b7lules de barrera hematoencef\u00e0lica, deixant menys LRP1 &#8220;portadors&#8221; disponibles. En contra, si s&#8217;uneix a molt poca A\u03b2, el senyal \u00e9s massa feble per activar el transport. En tots dos casos, el resultat \u00e9s el mateix: A\u03b2 s&#8217;acumula dins del cervell.<\/p>\n\n\n\n<p>Els f\u00e0rmacs supramoleculars desenvolupats en aquest treball actuen com un interruptor que restableix el sistema. En imitar els lligands de LRP1, poden unir-se a A\u03b2, creuar la barrera hematoencef\u00e0lica i iniciar el proc\u00e9s d&#8217;eliminaci\u00f3 d&#8217;esp\u00e8cies t\u00f2xiques del cervell. Dissenyades amb un enfocament <strong>d&#8217;enginyeria molecular <em>bottom-up<\/em> (de baix a dalt)<\/strong>, aquestes nanopart\u00edcules combinen un control de mida prec\u00eds amb un nombre definit de lligands a la superf\u00edcie, creant una plataforma multivalent capa\u00e7 d&#8217;interactuar amb els receptors cel\u00b7lulars d&#8217;una manera molt espec\u00edfica. En participar en el tr\u00e0fic de receptors en la membrana cel\u00b7lular, obren una forma \u00fanica i innovadora de <strong>modular la funci\u00f3 del receptor<\/strong>.
