M\u00e9s enll\u00e0 de l&#8217;impacte personal en la qualitat de vida i la independ\u00e8ncia, la p\u00e8rdua de visi\u00f3 suposa una <strong>c\u00e0rrega econ\u00f2mica global estimada en m\u00e9s de 400.000 milions de d\u00f2lars anuals <\/strong>en costos sanitaris i p\u00e8rdua de productivitat<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p><em>Aquestes mol\u00e8cules no curen la ceguesa. Tanmateix, s\u00f3n sorprenentment eficaces a l&#8217;hora de restaurar la visi\u00f3, i ho fan mitjan\u00e7ant un enfocament molt senzill i potencialment f\u00e0cil d&#8217;usar per als pacients<\/em><\/p>\n\n\n\n<p>Pau Gorostiza<\/p>\n<\/blockquote>\n\n\n\n<p>En moltes d&#8217;aquestes condicions, <strong>les c\u00e8l\u00b7lules fotoreceptores<\/strong> \u2014els detectors de llum de la retina\u2014 es <strong>degeneren progressivament i moren. <\/strong>Tot i que la resta del circuit neuronal de la retina roman en gran manera intacte i funcional, ja no rep els <strong>senyals de llum<\/strong> necessaris per dirigir el processament visual cap al cervell. Aquest fet representa una finestra d&#8217;oportunitat que ha impulsat noves vies de recerca per desenvolupar tractaments capa\u00e7os de restaurar la sensibilitat a la llum a l&#8217;ull. Les estrat\u00e8gies actuals inclouen la ter\u00e0pia g\u00e8nica \u2014efectiva nom\u00e9s per a un subconjunt molt petit de pacients amb mutacions espec\u00edfiques\u2014 i les pr\u00f2tesis electr\u00f2niques de retina, que s\u00f3n invasives, costoses i requereixen una formaci\u00f3 extensa per al seu \u00fas efectiu. M\u00e9s recentment, l&#8217;optogen\u00e8tica i els f\u00e0rmacs sensibles a la llum han entrat en assajos cl\u00ednics. Malgrat els resultats de seguretat alentidors en el cas dels f\u00e0rmacs sensibles a la llum, aconseguir una visi\u00f3 d&#8217;alta qualitat en nivells d&#8217;il\u00b7luminaci\u00f3 ambiental continua sent un gran desafiament.<\/p>\n\n\n\n<p>Ara, un consorci liderat per l<strong>&#8216;Institut de Bioenginyeria de Catalunya (IBEC) <\/strong>ha fet un gran pas endavant en la recerca de tractaments per restaurar la visi\u00f3 en humans. Publicada al <a href=\"https:\/\/www.doi.org\/10.1021\/jacs.5c18611\"><em>Journal of the American Chemical Society<\/em><\/a><em> (JACS<\/em>), la investigaci\u00f3 presenta una nova classe de <strong>f\u00e0rmacs fotosensibles <\/strong>capa\u00e7os de restaurar funcions visuals clau en models animals de ceguesa. Els compostos poden assumir el paper funcional <strong>dels fotoreceptors <\/strong>injectant-los a l&#8217;ull com es fa amb altres f\u00e0rmacs oft\u00e0lmics, o fins i tot administrant-los en forma de<strong> col\u00b7liris<\/strong>. En qualsevol cas, no requereixen manipulaci\u00f3 gen\u00e8tica ni dispositius implantats. A m\u00e9s, els compostos presenten <strong>perfils de seguretat<\/strong> prometedors que els posicionen com a possibles candidats a f\u00e0rmacs per a futures ter\u00e0pies de restauraci\u00f3 de la visi\u00f3.<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p><em>El nostre objectiu era restaurar la visi\u00f3 mitjan\u00e7ant un mecanisme molecular el m\u00e9s semblant possible al funcionament d&#8217;una retina sana<\/em>.<\/p>\n\n\n\n<p>Rosalba Sortino<\/p>\n<\/blockquote>\n\n\n\n<p><em>&#8220;Aquestes mol\u00e8cules no curen la ceguesa, ja que no aborden la causa de la degeneraci\u00f3 dels fotoreceptors. Tanmateix, s\u00f3n sorprenentment eficaces a l&#8217;hora de restaurar la visi\u00f3, i ho fan mitjan\u00e7ant un enfocament molt senzill i potencialment f\u00e0cil d&#8217;usar per als pacients&#8221;<\/em>, explica <strong>Pau Gorostiza<\/strong>, professor de recerca ICREA a l&#8217;IBEC, l\u00edder del <a href=\"https:\/\/ibecbarcelona.eu\/es\/nanoprobes\/\">grup de Nanosondes i Nanoconmutadors<\/a>, membre del CIBER-BBN i co-l\u00edder de l&#8217;estudi.\u00a0<\/p>\n\n\n\n<p><em>&#8220;El nostre objectiu era restaurar la visi\u00f3 mitjan\u00e7ant un mecanisme molecular el m\u00e9s semblant possible al funcionament d&#8217;una retina sana<\/em>&#8220;, afirma <strong>Rosalba Sortino<\/strong>, ex-doctoranda de la Universitat de Barcelona i actualment investigadora postdoctoral en el grup de Pau Gorostiza a l&#8217;IBEC i co-primera autora de l&#8217;estudi. <em>&#8220;En lloc de sortejar el processament visual de la retina, vol\u00edem reactivar-lo just en el mateix punt del circuit retinal on normalment actuen els fotoreceptors perduts&#8221;.<\/em><\/p>\n\n\n\n<p>El treball es va dur a terme en col\u00b7laboraci\u00f3 amb l&#8217;equip liderat per <strong>Pedro de la Villa<\/strong> a&nbsp; la <strong>Universitat d&#8217;Alcal\u00e1 (UAH<\/strong>), aix\u00ed com amb personal investigador de l<strong>&#8216;Institut de Qu\u00edmica Avan\u00e7ada de Catalunya (IQAC-CSIC),<\/strong> la <strong>Universitat de Barcelona (UB)<\/strong>, l<strong>&#8216;Institut Ram\u00f3n y Cajal de Recerca en Salut (IRYCIS), <\/strong>&nbsp;la <strong>Universitat Aut\u00f2noma de Barcelona (UAB)<\/strong> i la <strong>Fundaci\u00f3 Eduard Soler<\/strong>.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong><strong>Restauraci\u00f3 de la funci\u00f3 visual en models animals<\/strong><\/strong><\/h4>\n\n\n\n<p>L&#8217;enfocament es basa en la <strong>fotofarmacologia<\/strong>, una t\u00e8cnica que permet controlar de forma reversible l&#8217;activitat dels f\u00e0rmacs mitjan\u00e7ant la llum. Aquest innovador m\u00e8tode consisteix a modificar l&#8217;estructura qu\u00edmica dels compostos incorporant un interruptor molecular activat per la llum, la qual cosa permet regular la seva acci\u00f3 farmacol\u00f2gica. Amb aquest objectiu, l&#8217;equip d&#8217;investigaci\u00f3 va desenvolupar una fam\u00edlia de compostos anomenats <strong>prosthe6<\/strong> que es dirigeixen a les neurones bipolars ON i van restaurar amb \u00e8xit <strong>els moviments oculars sac\u00e0dics<\/strong> (reflex optocin\u00e8tic) en larves cegues de peix zebra, un model animal \u00e0mpliament utilitzat per estudiar l&#8217;agudesa visual. Per\u00f2 el m\u00e9s cridaner \u00e9s que els investigadors van demostrar la recuperaci\u00f3 del <strong>comportament innat d&#8217;evitaci\u00f3 de la llum<\/strong> en models murins de degeneraci\u00f3 macular associada a l&#8217;edat i de retinitis pigment\u00e0ria.<\/p>\n\n\n\n<p>Els ratolins sans prefereixen, de manera natural, romandre en ambients foscos i instintivament eviten les zones amb molta llum, un comportament que dep\u00e8n completament d&#8217;un sistema visual funcional. Els ratolins cecs, en canvi, perden aquesta prefer\u00e8ncia i es mouen indistintament entre espais de llum i foscor, ja que no poden percebre la llum. Despr\u00e9s del tractament amb prosthe6, els ratolins cecs van mostrar novament una prefer\u00e8ncia clara i espont\u00e0nia per les zones fosques, cosa que indica que podien percebre la llum i usar aquesta informaci\u00f3 per guiar el seu comportament. Aquesta recuperaci\u00f3 es va produir sense cap entrenament i sota nivells de llum comparables als que es troben en interiors o en un dia ennuvolat, demostrant que el tractament restaura la percepci\u00f3 funcional de la llum capa\u00e7 d&#8217;impulsar un comportament natural i guiat visualment.<\/p>\n\n\n\n<p>Dos compostos, <strong>prosthe6-12 i la prosthe6-15<\/strong>, van mostrar resultats especialment prometedors. Els comportaments restaurats es van observar no nom\u00e9s despr\u00e9s de la injecci\u00f3 intraocular, sin\u00f3 tamb\u00e9 despr\u00e9s de <strong>l&#8217;administraci\u00f3 t\u00f2pica en forma de col\u00b7liris<\/strong>.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\"><strong><strong>Actuar en el lloc adequat de la retina<\/strong><\/strong><\/h4>\n\n\n\n<p>Els compostos prosthe6 funcionen actuant sobre un tipus espec\u00edfic de c\u00e8l\u00b7lules de la retina anomenades <strong>c\u00e8l\u00b7lules bipolars ON<\/strong>, que normalment reben senyals dels <strong>fotoreceptors, <\/strong>les c\u00e8l\u00b7lules de l&#8217;ull encarregades de detectar la llum.<\/p>\n\n\n\n<p><em>&#8220;En la visi\u00f3 sana, les c\u00e8l\u00b7lules bipolars ON exerceixen un paper clau en la transmissi\u00f3 d&#8217;informaci\u00f3 sobre la pres\u00e8ncia de llum a la resta del circuit visual. En les malalties oculars degeneratives, tot i que es perden els fotoreceptors, gran part d&#8217;aquest circuit subjacent roman intacte, per\u00f2 inactiu. Aix\u00f2 crea una gran oportunitat terap\u00e8utica&#8221;, <\/em>explica Pedro de la Villa, co-l\u00edder de l&#8217;estudi.<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p><em>En les malalties oculars degeneratives, tot i que es perden els fotoreceptors, gran part d&#8217;aquest circuit subjacent roman intacte, per\u00f2 inactiu. Aix\u00f2 crea una gran oportunitat terap\u00e8utica<\/em>.<\/p>\n\n\n\n<p>Pedro de la Villa<\/p>\n<\/blockquote>\n\n\n\n<p>En actuar sobre una prote\u00efna (mGlu6) present en aquesta part preservada de la retina, els compostos prosthe6 poden assumir la funci\u00f3 dels fotoreceptors que s&#8217;han perdut. Quan la llum entra a l&#8217;ull, aquestes mol\u00e8cules responen canviant de forma, cosa que desencadena senyals a la retina d&#8217;una manera molt similar al de la visi\u00f3 natural. D&#8217;aquesta manera, els f\u00e0rmacs actuen com a veritables <strong>\u00abpr\u00f2tesis moleculars\u00bb<\/strong>, ajudant l&#8217;ull a tornar a processar la llum sense necessitat d&#8217;implants ni modificacions gen\u00e8tiques.<\/p>\n\n\n\n<p>\u00c9s important destacar que aquests compostos estan dissenyats per funcionar sota condicions normals d&#8217;il\u00b7luminaci\u00f3 i no requereixen dispositius que millorin la llum com passa en el cas de l&#8217;optogen\u00e8tica. S\u00f3n mol\u00e8cules petites i solubles en aigua que responen a la llum visible o blanca ordin\u00e0ria, com la il\u00b7luminaci\u00f3 interior o la llum di\u00fcrna, sense necessitat de fonts de llum intenses o especialitzades.<\/p>\n\n\n\n<figure class=\"wp-block-embed is-type-video is-provider-youtube wp-block-embed-youtube wp-embed-aspect-16-9 wp-has-aspect-ratio\"><div class=\"wp-block-embed__wrapper\">\n<iframe title=\"Eyelumina | molecular photoswitches to restore vision\" width=\"1200\" height=\"675\" data-src=\"https:\/\/www.youtube.com\/embed\/PCgXsnKuQ0s?feature=oembed\" frameborder=\"0\" allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture; web-share\" referrerpolicy=\"strict-origin-when-cross-origin\" allowfullscreen src=\"about:blank\" class=\"lazyload\" data-load-mode=\"0\"><\/iframe>\n<\/div><\/figure>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h4 class=\"wp-block-heading\"><strong><strong>De la qu\u00edmica fonamental a la ci\u00e8ncia translacional<\/strong><\/strong><\/h4>\n\n\n\n<p>El moment en qu\u00e8 es publica aquest estudi tamb\u00e9 \u00e9s rellevant, ja que arriba poc despr\u00e9s de la publicaci\u00f3 del <strong>primer assaig cl\u00ednic en humans d&#8217;un fotof\u00e0rmac per a la restauraci\u00f3 de la visi\u00f3 <\/strong>(dirigit a una prote\u00efna diferent), cosa que posa de manifest que aquesta estrat\u00e8gia terap\u00e8utica emergent comen\u00e7a a obrir-se cam\u00ed cap a la seva aplicaci\u00f3 en pacients.<\/p>\n\n\n\n<p>La tecnologia prosthe6 est\u00e0 <strong>protegida per una patent<\/strong> i els investigadors estan avaluant ara la seva seguretat i formulaci\u00f3 per prolongar la durada de la rehabilitaci\u00f3 visual. L&#8217;equip tamb\u00e9 est\u00e0 treballant amb <strong>Eyelumina, una empresa derivada <\/strong>en proc\u00e9s de constituci\u00f3, per assegurar inversions que donin suport al desenvolupament translacional i futurs assajos cl\u00ednics.<\/p>\n\n\n\n<p><em>\u00abConvertir aix\u00f2 en una ter\u00e0pia \u00e9s un proc\u00e9s llarg i complex\u00bb, afirma Gorostiza. \u00abPer\u00f2 els resultats mostren que hi ha una possibilitat realista de restaurar una visi\u00f3 d&#8217;alta qualitat mitjan\u00e7ant f\u00e0rmacs, de forma no invasiva, reversible i amb un mecanisme independent de la malaltia retinal o de la mutaci\u00f3 gen\u00e8tica concreta, cosa que permetria arribar a la majoria dels pacients\u00bb.<\/em><\/p>\n\n\n\n<p>En cas de tenir \u00e8xit en humans, aquest enfocament basat en f\u00e0rmacs oferiria una <strong>alternativa \u00e0mpliament accessible i assequible<\/strong> a les tecnologies existents de restauraci\u00f3 de la visi\u00f3, especialment rellevant per a pacients amb degeneraci\u00f3 avan\u00e7ada de la retina per als quals actualment no existeixen tractaments efectius.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<p><strong><strong>Article de refer\u00e8ncia<\/strong><\/strong><\/p>\n\n\n\n<p>Rosalba Sortino, Aleix Gonz\u00e1lez-D\u00edez, Santiago Milla-Navarro, Joaqu\u00edn Mart\u00ednez-Tambella, V\u00edctor Paleo-Garc\u00eda, Eric Calatayud, Paula de Saralegui, Ekin Opar, \u00c0lvar Claparols, Josecarlo A.
